Capture/Replay: Remove references to uniform block indexes. This removes the references in captured traces to the uniform block index map. Because uniform block indexes use a double indirection, we store them in an unordered map. Moving to plain C precludes all uses for templated types, so we need to funnel the calls through a custom function.
