Vulkan: Remove Subject/Observer from BufferImpl
Right now the gl::Buffer back end send message to the front end and then
gl::Buffer propagate the message to the observers (vertex array,
textures, transform feedback). We are seeing many of these kind of
message passing (mainly to vertexArray), and each message is a virtual
function call. The message call also lacks of context information that
we can not do certain optimizations. This CL adopts the new API feedback
argument approach for buffer APIs from the back end to the front end.
The only difficulty I ran into is D3D backend where the message could be
delivered from draw calls. For now the subject/observer code path is
still kept in the gl::Buffer, but no back end will use it except D3D11.
That will be removed in the later CL when D3D11 switch to use feedback
mechanism.

Track WebGL buffer type in gl::Buffer state.
WebGL has special validation that disallows buffers from changing
"WebGL buffer type". Once a buffer is marked as having index data it
cannot be bound as any other type of buffer except the copy binding
points to copy to other index buffers. This disallows any GPU writes
to buffers used for index data.
Use this rule to shadow index buffer data when the driver is unable
to give us robust access. Index range computation can be done much
faster when the buffer does not need to be read back.

GL: Ensure all instanced attributes have a buffer with data
Apple OpenGL drivers sometimes crash when given an instanced draw with
a buffer that has never been given data.
It's not efficient to check if the attribute is both zero-sized and
instanced so just ensure that every time a zero-sized buffer is bound
to an attribute, it gets initialized with some data.

Use packed enum for DrawElementsType.
The packing and unpacking take a few extra instructions. But it
completely obviates the need for any switches in the validation code.
Speed is slightly faster or the similar depending on the back-end.
Also add gl_angle_ext.xml to GL entry point generator inputs. This was
missing and would cause the code generation to miss certain changes.

Pass gl::Context to impl methods instead of ContextImpl.
In some cases we might have to call back into the GL layer, passing
the Context, and if we just have a ContextImpl pointer this isn't
possible. It also removes the need for SafeGetImpl.

D3D11: Work around Intel uniform buffers bug.
When copying from a staging buffer to a uniform buffer, the first
upload would be incorrect. Work around this by trying to upload
directly to a uniform buffer on the first BufferSubData call.

Allow BufferGL to function without being able to map buffers for read.
Reading back buffer data is required for index range validation but
without glMapBufferRange it is not possible to read back buffer data on
OpenGL ES. To work around this, keep a shadow copy of the buffer data
when this function is not available.

Store index ranges in a new struct that tracks how many real indices were
seen. Update index caching to key on primitive restart being enabled and
update index counting functions to skip primitive restart indicies when
needed.

Add a Buffer::getIndexRange function.
Instead of exposing the index range cache, add a more generic method that
allows the buffers to handle their own caching of index ranges.
BufferImpl::getData can be hard to implement for BufferGL because there
isn't a way to tell the buffer to unmap and glGetBufferSubData can be very
expensive, requiring an extra copy of the data.

Replace non-copyable macro with a helper class.
This class provides a simpler scheme for blocking default copy
and assignment operators. It also reduces the amount of code
needed since it's inherited to child classes. This also fixes
the conflict between our macro and the same-named macro in
Chromium code.
