implemented streaming of RelaxNG (when possible) on top of the xmlReader
* relaxng.c xmlreader.c xmllint.c include/libxml/relaxng.h
include/libxml/xmlreader.h: implemented streaming of
RelaxNG (when possible) on top of the xmlReader interface,
provided it as xmllint --stream --relaxng .rng .xml
This seems to mostly work.

After coming to the conclusion that the original RelaxNG validation code

rewritten to use the derivation algorithm from James Clark and
redebugged it (nearly) from scratch:
found 373 test schemas: 372 success 1 failures
found 529 test instances: 529 success 0 failures
