Implement OES_gpu_shader5
Based on the specs, there is no difference between the OES version
and the EXT version.
* Added support for use in shaders
* Added test to make sure the OES extension works.
* Turned the repeated test code into a function:
* testArrayOfArrayOfSamplerDynamicIndex()
