  thread: Fix compilation error on IRIX.
  
  * lib/glthread/thread.h: Include <unistd.h>. Include <signal.h> when
  needed; include it outside the C++ extern "C" {} block.
  * doc/posix-headers/pthread.texi: Mention the problem with
