reader: Simplify error handling
Only use structured error handlers for parser, Schemas and RelaxNG
contexts. Also use structured error handler for XInclude context.
Remove TODO macro.

tests: Start with testparser.c for extra tests
Several issues require customized tests. Start with a test that push
parses large documents. See #539.
